What is an anal fissure?

An anal fissure is a cut or tear in the anus, extending upwards into the anal canal, and usually cause severe pain during bowel movements. Fissures affect men and women equally and occur at any age. In infancy, anal fissure is the most common cause of rectal bleeding.

Anal fissures occur in the anoderm, a specialised tissue lining the anus and anal canal. Unlike skin, anoderm has no hairs, sweat glands, or sebaceous (oil) glands and contains a larger number of somatic nerves similar to nerves on the skin. These nerves sense light touch and pain, which explains why anal fissures are so painful.

What are the symptoms of anal fissures?

Those with anal fissures typically experience anal pain that worsens with bowel movements. The rectal pain following a bowel movement may be brief or long lasting and subsides between bowel movements. Bleeding in small amounts, itching and a foul-smelling discharge may also occur due to the discharge of pus from the fissure.

What causes anal fissures?

After a bowel movement, the internal anal sphincter does not relax as normal. Instead, it contracts even more for a few seconds before it goes back to its elevated resting level of contraction. It is believed that this high resting pressure and the "overshoot" contraction of the internal anal sphincter following a bowel movement, pulls the edges of the fissure apart and prevents the fissure from healing.
