What is postherpetic neuralgia?

After a shingles outbreak has subsided, about 20% of people will experience pain in the nerve endings. This condition is called postherpetic neuralgia (PHN), and the pain can recur at unknown intervals.

PHN pain is neurogenic, resulting from peripheral nerve injury, and tends to occur in the same site as the shingles. The affected area is often extremely sensitive, with even loose clothing causing excruciating pain. PHN usually lasts a few months, but in a small percentage of people, PHN can last for several years.

Until recently, PHN has been difficult to treat, as anti-viral drugs and other medications (including corticosteroids and local/regional anaesthesia) had been tried without much success. PHN treatment is based on managing the neuralgia once it has developed, but because of the neurogenic character of the pain, the value of conventional analgesics is limited. What causes shingles?
