What is postherpetic neuralgia?

About 20% of people who have suffered from shingles will experience residual pain in the nerve endings after the outbreak has subsided. This condition is called postherpetic neuralgia, or PHN. The pain often returns intermittently at unknown intervals.

The pain of PHN is neurogenic, resulting from peripheral nerve injury, occurring around the site of the initial shingles episode. The affected area is often sensitive to any stimuli, and even clothing can cause unbearable pain. Spontaneous remission of PHN occurs in many patients within a few months, and in a small percentage, the pain can last for several years.

Until recently, PHN was difficult to treat. Anti-viral drugs and other medications such as corticosteroids and local/regional anaesthesia had been tried without much success. Treatment is based on managing the neuralgia once it develops, but the value of conventional analgesics is limited because of the neurogenic character of the pain, and sufferers often rely on anti-depressants, which do not address the problem.

Anti-epileptics have also been used, but their value in PHN is unclear and their adverse effects make them unsuitable for the elderly. However, acute PHN conditions are often eased immediately with PHN No More, which is an anaesthetic, sedative treatment that provides great relief to those suffering from PHN.

What are the signs and symptoms of PHN?

If your shingles rash has healed, but you are experiencing pain in the area where the shingles rash occurred, it could be PHN. In addition, if touching the skin causes pain, the patient may be experiencing allodynia, a frequent symptom of PHN. Allodynia is a condition in which the skin is unusually sensitive to normally painless stimuli, such as the touch of clothing or a gust of wind.

What causes shingles?

PHN occurs after a person has suffered from shingles. Shingles is caused by a reactivation of the varicella-zoster virus (VZV), the virus that causes chickenpox. Anyone who has had chickenpox can get shingles, although it is most common in individuals over the age of 50. It is estimated that ten to twenty per cent of all Americans – and fifty per cent or more of individuals aged 85 and older – will experience shingles during their lifetime. Individuals who have existing conditions, or are undergoing medical treatment, that weaken their immune systems are also more likely to develop shingles. These include: HIV; chemotherapy or radiation therapy; corticosteroids; transplant operations and possibly stress.
