Hemorrhoids are vascular structures in the anal canal, composed of blood vessels and connective tissue. Helping with stool control, they act as "cushions". Everyone has hemorrhoids, and until they become enlarged and inflamed, they are often unnoticed.
A rich network of arteries descends from the rectum to the anal canal, supplying arterial blood to hemorrhoidal blood vessels. This network passes through the hemorrhoidal cushions, which is why bleeding from Hemorrhoids is bright red (arterial blood) rather than dark red (venous blood), and why hemorrhoidal bleeding can be severe.
The anal veins drain blood away from the anal canal and the hemorrhoids in two directions. The first is upwards into the rectum (the last part of the large intestine leading to the anus), and the second is downwards, beneath the skin surrounding the anus.
There are two types of hemorrhoids:
A prolapsed hemorrhoid occurs when an internal Hemorrhoid bulges into the anal canal, occasionally protruding from the anus. A prolapsed hemorrhoid can be pushed back inside with a finger, but will prolapse again with the next bowel movement.
The anal canal contains two types of nerves, the visceral nerves (rectal side) and somatic nerves (near the anus). Visceral nerves can only sense pressure, whereas somatic nerves can sense pain. Therefore, internal hemorrhoids, which are closer to the rectum, are usually painless.
Hemorrhoids are exposed to stools that pass through the anal canal. When stools are hard, this exposure may cause bleeding, and sometimes pain, and if stools leak onto the anal skin, this may cause itchiness. The rectal lining also secretes mucous, which moistens the anus and surrounding skin, which can cause increased anal itchiness.
Most people notice internal Hemorrhoids when they see drops of blood in the toilet bowl or on the toilet paper, whereas external hemorrhoids can be felt when people wipe themselves after a bowel movement. If blood clots inside the hemorrhoid (thrombosis), the person feels tenderness, burning, itching or pain.
Hemorrhoid symptoms often only last a few days, but consult your doctor immediately if symptoms persist or if you have severe pain or bleeding.
Internal hemorrhoids form when blood vessels in the anal cushions swell. This causes the supporting hemorrhoidal tissue to increase until the enlarged mass of tissues and blood vessels protrudes into the anal canal. It is still unclear why hemorrhoids enlarge, as although there are several causes, the reason for the swelling is unknown.
Hemorrhoids are common in both men and women, but they are frequently seen in those between 45 and 65 years of age.
