A wart is a local growth in the upper skin layer (epidermis) caused by the human papillomavirus (HPV) infection. Although warts are transmitted by skin contact, it can often only affect one family member. A person with warts can auto-inoculate themselves by touching a wart then touching an unaffected area on their body. However, warts usually appear on just one part of the body without spreading.
Over 100 types of HPVs infect humans, with more than forty HPV types infecting the genital tract and anus, causing genital warts. Genital (venereal) warts transmit through sexual contact, labelling them as a sexually transmitted disease (STD). The HPV types infecting skin cause warts on other parts of the body. For example, "common warts" occur on the hands or on top of the feet. A wart on the sole of the foot is a plantar wart and can be very painful.
Common warts (verruca vulgaris): These warts are firm, thickened bumps, with a rough surface, sometimes resembling a cauliflower. These warts often have tiny dark spots, which form from clotted blood vessels. Common warts usually appear on the knuckles, fingers, elbows and knees, although they may appear anywhere on the body.
Verrucae (plantar warts): Plantar warts appear on the soles of the feet, and occasionally the heel and toes. Plantar warts can be extremely painful, partly because they usually grow back into the skin as the person's weight pushes onto the foot sole. Plantar warts often have a black dot in the middle, with a surrounding hard, white area. The dark dot is the wart's blood supply.
Plane warts (verruca plana): Plane warts, also called flat warts, are round, flat and smooth. They are generally yellowish, brownish or skin colour. Plane warts usually appear on the hands, legs and face. These warts are more common in young children and are rare in adults.
Filiform warts (verruca filiformis): Filiform warts are long and usually appear on the eyelids, neck and armpits.
Mosaic warts: Mosaic warts grow in clusters, and palmar warts are mosaic warts that grow on the palm of the hands and feet.
Genital warts (condylomata acuminate): Usually, genital warts do not have symptoms, but they are sometimes associated with itching, burning, or tenderness. They may result in localised irritation, depending on their location. Women with genital warts inside the vagina may experience bleeding following sexual intercourse or an abnormal vaginal discharge.
The HPV infection is common, and each year HPV infects an estimated 6 million people in the US. Around half of those are between the ages of 15 and 25. HPV does not usually cause any visible symptoms, as it remains inactive or concealed inside the body. Therefore, a large number of people, young or old, may have HPV without ever knowing it. However, even in its inactive state, the virus remains highly contagious. Thus, people may be unknowingly transmitting it to other people.
All warts spread through direct skin-to-skin contact, especially when infected areas touch damaged skin of an uninfected area. Damaged skin can be any small opening such as those caused by:
Skin exposed to excessive moisture, such as on sweaty feet, is at a higher risk of the HPV infection. A person with scratches or cuts on the soles of his/her feet can catch the infection by walking barefoot around swimming pools, gyms, or by sharing the socks or shoes of an infected person.
